- What Type of License do I Need in Order to Join Your Network?

CBH contracts and credentials behavioral health clinicians and specialists in our national networks. In the state of Georgia, CBH has credentialing criteria for the following licensure levels:
MD, DO: Psychiatrist
MD, DO: Addictionologist
PhD, PsyD, EdD: Clinical Psychologist
ANP, ARNP: Psychiatric Nurse Practioner
LPC: Licensed Professional Counselor
LCSW: Licensed Clinical Social Worker
LMFT: Licensed Marriage and Family Therapist - Do I need a Medicaid Number to Join the CBH Network?
- Yes, CBH requires that you obtain a Medicaid number for reimbursement purposes.
- How Do I Get a Medicaid Number?

You must first attempt to apply on the State website. At the main page, choose the "Provider Information" tab. Once you complete the application, you will receive a confirmation number. The State will notify you when your application has been approved and your Medicaid number is active with the State. Please notify CBH once you obtain your notification of approval.

- How do I Join the CBH Provider Network?
- It's easy! Simply call CBH Network Development at (800) 947-0633. Your local Network Development Manager will provide you with an Agreement and all applicable credentialing materials. Or simply complete the Join Our Network Form and a Network Manager will forward the appropriate materials to you.

Participating providers are able to conduct an evaluation and 5 follow up visits for outpatient treatment services without prior authorization required. Providers may simply submit claims for these treatment visits. For authorizations for treatment sessions beyond the initial 6, an Outpatient Treatment Request (OTR) form must be submitted by providers. Authorization requests may be submitted via mail, fax, or web.
For psychological testing, prior authorization is required. Participating physicians and nurse practitioners do not need prior authorization for medication management visits. - What if I Need Additional Training or Orientation?

In an effort to provide the best customer service and meet your needs, CBH offers regional Network Development representation. Your dedicated regional Network Development Manager is available to meet with you to conduct face-to-face meetings and seminars at your convenience.
CBH will also host a variety of provider education sessions, Q&A forums, and a number of conferences. Please contact your regional Network Development Manager for further details.
